Portal Access Terms and Conditions

Intended Users

This portal is for prescribers and their staff who have a relationship with Coastal Pharmacy & Wellness. Use by others is prohibited and subject to revocation of site membership.

Access is limited to:

• Licensed prescribers (physicians, nurse practitioners, physician assistants, and other practitioners with prescriptive authority) currently in good standing with their licensing board, and
• Staff members acting on behalf of, and at the direction of, a registered prescriber’s practice.

Coastal Pharmacy & Wellness reserves the right to verify prescriber status (including via NPI lookup and other public licensure databases) and to approve, deny, or revoke access at its sole discretion.

Account Registration

By registering for portal access, you represent and warrant that:

• All information you submit (including name, practice name, NPI, and contact information) is accurate, current, and belongs to you or your practice;
• You are authorized to register on behalf of the practice named in your registration, if applicable; and
• You will promptly update your account information if it changes (e.g., change of practice, licensure status, or contact details).

Accounts are non-transferable. Sharing your login credentials with anyone outside your practice, or with any individual not independently eligible for portal access, is prohibited and may result in immediate termination of access.

Registrations are subject to an internal review process. Coastal Pharmacy & Wellness may place an account in pending status, request additional verification, or decline registration for any reason, including where prescriber status cannot be confirmed.

No Protected Health Information (PHI) on This Portal

This portal is for reference materials and blank prescription forms only. Do not submit, upload, or transmit any patient information including PHI through this website, its registration forms, or any contact form on this site.

• Completed prescriptions must be filled out, printed, signed, and faxed to Coastal Pharmacy & Wellness at the fax number provided on each form, or submitted through the secure channels Coastal Pharmacy & Wellness separately establishes for that purpose.
• Any patient-specific or clinical information exchanged with Coastal Pharmacy & Wellness outside of this portal (e.g., by phone or fax) is governed by our standard pharmacy privacy policy and HIPAA practices, not by these Portal Access Terms.

Acceptable Use

You agree not to:

• Use the portal for any purpose other than accessing prescription forms, formulation guides, clinical reference materials, or other content Coastal Pharmacy & Wellness makes available to prescribers;
• Attempt to access accounts, tiers, or content you are not authorized to use;
• Copy, resell, republish, or distribute portal content to any third party outside your practice without written permission;
• Use automated tools (bots, scrapers, etc.) to access or download portal content in bulk;
• Interfere with the security, integrity, or normal operation of the portal.

Clinical & Reference Content Disclaimer

Formulation guides, dosage forms, sample formulas, studies, whitepapers, and any other clinical or educational content made available through this portal are provided for reference purposes only.

• Compounded formulations described on this site are not FDA-approved.
• Content on this portal does not constitute medical advice and is not a substitute for independent clinical judgment.
• Sample formulas reflect prescriptions previously written by licensed prescribers and are provided as a starting reference; the prescriber remains solely responsible for the appropriateness of any prescription written for a specific patient.
• Coastal Pharmacy & Wellness makes no representation or warranty as to the safety, efficacy, or suitability of any formulation for any particular patient.

Intellectual Property

All content on this portal — including forms, guides, formulas, logos, and written materials — is the property of Coastal Pharmacy & Wellness or its licensors and is provided solely for use by authorized portal members in connection with their prescribing relationship with Coastal Pharmacy & Wellness. No other license or right is granted.

No Warranty; Limitation of Liability

This portal and its content are provided “as is” without warranties of any kind, express or implied. To the fullest extent permitted by law, Coastal Pharmacy & Wellness disclaims all liability arising from your use of, or reliance on, this portal or its content, including any errors, omissions, or interruptions in availability.

Account Termination

Coastal Pharmacy & Wellness may suspend or terminate portal access at any time, without notice, for any reason, including suspected misuse, inaccurate registration information, loss of licensure or prescriptive authority, or violation of these terms.

You may request closure of your account at any time by contacting us using the information below.

Changes to These Terms

We may update these Portal Access Terms from time to time to reflect changes in the portal, our practices, or legal requirements. Continued use of the portal after changes are posted constitutes acceptance of the revised terms. Material changes will be reflected in the “Last updated” date above.

Governing Law

These terms are governed by the laws of the State of Maine, without regard to its conflict-of-laws principles.
