Prescription forms and downloads are available to registered providers and their office staff. Have your NPI and practice details handy — we verify each request by hand, and most are approved the same business day.
Office staff get their own logins under your practice, approved by the provider on file. No shared passwords.

Who can register
The portal is available to prescribers and practice staff. Access will be granted based on NPI registration (individual NPI for prescribers and practice NPI for staff).
